About our blood tests

Prometheus is a CLIA laboratory that performs tests that have been ordered by a healthcare provider. Patients can have their blood sample taken through a mobile phlebotomy service or at a number of laboratories (sometimes called draw sites) across the country. The blood sample is then sent to our laboratory in San Diego, California, to perform the test the healthcare provider ordered. Contact Client Services for more information.
In most cases, you do not need to do anything to prepare for a PROMETHEUS® IBD sgi Diagnostic™, PROMETHEUS® Anser™ ADA, or PROMETHEUS® Anser™ IFX test. Always check with your healthcare provider to find out about anything he or she may want you to do before your blood test.
Most blood test results are available to your healthcare provider within a week from when we receive your sample. Samples usually reach us within 24 hours of being collected. But it often takes a few days to process all the patient, healthcare provider, and test information. You can contact Client Services to check the status of your test results. However, the actual test results are available only to the healthcare provider who requested the test. He or she will share the results with you.
All results are sent to the healthcare provider who requested the test.

Insurance and billing

We offer a variety of testing services that are not available at other laboratory facilities. Contact our billing department for pricing and payment options.

We accept payment by check, money order, and most major credit cards. Payment for blood tests is due when you receive our invoice. Each blood test will have a unique patient reference number. To ensure that your payment is processed correctly, please include the patient name and patient reference number(s) with your payment. Credit card payments can be processed by phone, fax, or mail, using our Credit Card Authorization Form. Payment by mail should be sent to:

Prometheus offers a variety of services and support programs to patients with all types of insurance. These services and programs include:

  • The PROMETHEUS® Links™ Program:
    • Will file multiple appeals on the patient’s behalf and, if the patient’s insurance still denies the claim, may cap the out-of-pocket costs at $250
    • Helps remove the remaining cost after the patient’s insurance company has paid its part of the charges (the allowable)
    • May further reduce out-of-pocket costs based on the patient’s income level
  • Self Pay Program:
    • Offers a 25% discount to patients who do not have insurance coverage
  • Other:
    • No-interest payment plan available
    • Potential discounts off balances (not inclusive of co-pays, deductibles, or coinsurance)

Yes. We are a Medicare participating provider.

Your insurance company will process a claim for our testing services based on your insurance coverage. The insurance company may require a prior authorization or referral before you have the blood test. Prior authorization means that your insurance company has to approve the need for the blood test before you have the test. However, a prior authorization does not guarantee that the test will be covered or reimbursed. Also, prior authorization might not be necessary depending on which Prometheus test you have or where you go to have the test done. Please contact our billing department for more information.

We cannot guarantee reimbursement from your insurance company. You may be responsible for any unpaid balances. Payment is due when you receive our invoice. Our laboratory testing services may require some payment directly from you. This is called anout-of-pocket payment. Out-of-pocket payments include deductibles, co-pays, coinsurance, and noncovered services. We do have agreements with some insurance plans and laboratories across the country that may help lower your out-of-pocket payments. Contact Client Services for more information.

We will bill your primary insurance as long as we have been given complete and accurate insurance information, including a diagnosis code given by your healthcare provider. This includes the subscriber’s (insured’s) name, date of birth, and plan identification number; the employer name and group number; and the insurance claim address and phone number. We ask that you provide us with a clean copy of your insurance card (front and back) whenever possible. We do not automatically submit a claim to your secondary insurance carrier, but we can bill it if we have your complete insurance information. Our billing department is available to assist you with your claim and payments.

We will send you a courtesy letter to let you know that we intend to file a claim with your insurance company for you. You will receive a separate letter for every blood test you have. Each test will have a unique reference ticket number. This number is helpful when corresponding with us. Please review the information you receive from us. If you have any questions, contact our billing department.

If you receive an Explanation of Benefits (EOB) from your insurance company at any time, please forward a copy of this document to our billing department. This will help us assess whether you are eligible for our financial assistance programs.

Please contact our billing department with your invoice and any Explanation of Benefits (EOB) from your insurance company. We offer many support programs, and we can file appeals on your behalf for PROMETHEUS® Anser™ ADA and PROMETHEUS® Anser™ IFX tests. We also offer sample letters of correspondence and other materials to help you with the insurance process. If you have questions, contact our billing department.Your insurance provider may require that you complete an informed consent in order for us to file an appeal.

It is possible that insurance payments and Explanation of Benefits (EOB) may be sent directly to you from either your primary or your secondary insurance carrier. If this happens, sign the back of the insurance check. Underneath your signature, write “Deposit Only: Prometheus Laboratories Inc.” and forward the check to our billing department. You may also deposit the insurance check and send us a personal check for the amount owed. It is important that you forward a copy of your insurance EOB in order for us to process your payments correctly.
